hey artist get interviewed questionnaire v2.0

This questionnaire gives us something to start with and won't be published. 

If you haven't yet, please sign in/sign up before you start this questionnaire. Don't worry, making an account doesn't expose you to any spam or marketing-- just keeps trollers from clogging our site. (The link's over at the right.)

Please answer as completely as you can, since the more we understand what interests you, the happier you'll be with the interview. Depending on how you answer this first page, you will be asked to respond to questions on 1-5 more pages.  All the questions are optional after this first page. 

The whole questionnaire will probably take you about 10-30 minutes to complete, depending on how much you choose to write.  After you complete the questionnaire, we will contact you at the address you provide to schedule the next step.
